45 Day Review
Our professional nurses conduct thorough resident assessments every 45 days to evaluate each individual’s health status and care needs. These assessments are critical in developing personalized care plans that address the specific requirements of each resident, ensuring they receive the best possible care. By identifying changes in conditions promptly, we can adjust care plans to meet evolving needs.
Back Up CM/DN
Understanding that continuity of care is crucial, we ensure that a backup Case Manager or Delegating Nurse is always available to step in when the primary nurse is not available. This ensures that there is no disruption in the high level of care our residents receive. Our backup professionals are fully briefed on each resident’s needs and care plans, ensuring seamless service and peace of mind for residents and their families.
